- Sphere (SPH). For long sighted it is (+) and for short sighted (-).
- Cylinder (CYL). Cylinder is the amount of visual distortion caused by the irregular shape of the cornea.
- Axis. Axis is the direction of the astigmatism in degrees.
- Prism. Prism is the correction to balance the eyes.
- Add or Near. Add is the amount of addition for the correction needed to focus on close objects.
- Intermediate (INT). Intermediate is the amount of correction needed for intermediate distances.
